Why the technique matters

Less disruption than a traditional approach

Traditional breast augmentation is not one single operation, but many conventional techniques create the implant pocket through direct dissection and may place part or all of the implant beneath the pectoralis muscle. Preservé uses a prepectoral tissue plane, tumescent fluid, an atraumatic channel separator, and controlled balloon expansion. The goal is to create room for the implant while leaving more of the muscle, ligaments, nerves, blood vessels, and natural breast structure intact.

Preserves the chest muscle

The implant is placed above the pectoralis muscle, so the technique does not require the muscle release used in many dual-plane or subpectoral augmentations.

Creates the pocket by expansion

A channel separator and inflatable balloon gently elongate the tissue to form a measured pocket instead of relying on broad cutting or separation.

May support an easier early recovery

Less muscle and soft-tissue disruption may mean less tightness and a faster return to light daily activity. Recovery still varies, and normal restrictions remain important.

What is different in the operating room

Preservé changes how the implant pocket is made. It does not remove the usual breast implant risks, and it is not automatically the right technique for every body or every goal.

Traditional augmentation includes several valid techniques. The right comparison depends on your anatomy, implant size, placement, and whether a lift or other procedure is needed.

Early U.S. evidence

Promising recovery data, with important limits

A 2026 prospective series followed 159 patients treated at five U.S. surgical centers. Among the 90 patients who answered the recovery survey, 81% reported returning to normal daily activity within five days. Most procedures used local anesthesia with sedation, although general anesthesia was still used in 36% of cases.

The study was designed to describe the technique and its early outcomes. It did not directly compare Preservé with a traditional augmentation group, and average follow-up was just under five months. The results support Preservé as a less-invasive option, but they do not prove that it is better for every patient or predict an individual recovery.

Is Preservé right for you?

A strong option for the right anatomy and goals

Preservé may be worth considering if you want a proportionate increase in volume and prefer an approach that avoids cutting the chest muscle. It can be especially appealing when maintaining natural movement and limiting early disruption are priorities.

A traditional technique may offer more flexibility if you want a larger size change, need major correction of asymmetry or breast-fold position, have limited tissue coverage, or need a more extensive lift. Dr. Ambani will compare the approaches rather than assume Preservé is right for everyone.

Motiva combines its SmoothSilk shell surface with model-specific silicone gels. These are design features, not a promise of a better result or freedom from complications.

Different gel behavior

Round and Ergonomix use different gel properties to create their characteristic profiles. Implant choice is based on the full plan, not the gel alone.

Safety and follow-up

Breast implants require long-term care

Breast implants are not lifetime devices. The longer they are in place, the greater the chance of a complication or another operation.

Risks to discuss

Possible complications include capsular contracture, rupture, infection, fluid collection, pain, changes in sensation, asymmetry, malposition, visible rippling, scarring, and the need for revision or removal. FDA labeling also discusses breast implant-associated anaplastic large cell lymphoma, called BIA-ALCL, and reports of systemic symptoms sometimes described as breast implant illness.

Screening for silent rupture

For silicone gel implants, FDA-approved patient labeling recommends a first ultrasound or MRI five to six years after surgery, then every two to three years. An MRI is recommended if you have symptoms or if an ultrasound is unclear.

Routine breast care

Continue age-appropriate breast screening and tell the imaging center that you have implants. Before surgery, you will review the FDA patient decision checklist and the implant labeling with your surgeon.

What patients ask about Motiva Preservé

Is Preservé the implant or the surgical technique?

Preservé is the tissue-preserving surgical technique. Motiva SmoothSilk Round and Ergonomix are the FDA-approved implants. The implant and the method used to create its pocket are related decisions, but they are not the same thing.

Does less invasive mean risk-free?

No. Preservé still involves surgery and a breast implant. Infection, bleeding, scarring, changes in sensation, implant malposition, capsular contracture, rupture, and future operations remain possible.

Will I need general anesthesia?

Not necessarily. Preservé can be performed with local anesthesia and sedation in selected patients, but the safest anesthesia plan depends on your health, anatomy, comfort, and whether another procedure is being performed.

How quickly can I return to normal activity?

Early published data suggest many patients return to light daily activity within several days. That is not the same as being fully healed. Exercise, lifting, and other activities must follow Dr. Ambani's recovery instructions.

Are Motiva implants FDA approved?

Yes. Motiva SmoothSilk Round and SmoothSilk Round Ergonomix were approved by the FDA in 2024 for primary and revision breast augmentation in women age 22 and older.

How do I choose between Round and Ergonomix?

The choice depends on your measurements, tissue coverage, desired shape, and the implant size and profile that fit your body. Dr. Ambani will compare those factors with you during consultation.

Do breast implants need to be replaced every ten years?

There is no automatic replacement date, but implants are not lifetime devices. You may need another operation if a complication develops or if your goals change.

Are there alternatives to implants?
